Montefiore Nyack Hospital’s Diabetes Self-Management Education Training Program Re-Accredited

Nyack, N.Y. April 7, 2015 - Montefiore Nyack Hospital’s Diabetes Self-Management Education Training Program has earned reaccreditation from the American Association of Diabetes Educators (AADE) through its Diabetes Education Accreditation Program. To achieve four-year accreditation, organizations and programs must meet national standards and demonstrate excellence in providing diabetes-management education. Nyack’s Self-Management Education Training Program underwent voluntary evaluation by the AADE. The AADE reviewed the program’s structure, staff, clinical data, outcomes and quality improvement plans.

Montefiore Nyack Hospital’s team of certified diabetes educators offers self-management training for individuals affected by Type 1, Type 2, and gestational diabetes. “There is no cure for Diabetes, but it can be managed,” says Linda Suarez, MS, RN, CDE, manager of community education at Montefiore Nyack Hospital. “We work with individuals who are newly diagnosed or who have been dealing with the disease for some time, offering the latest in diabetes education and supports all designed to empower people to take control of their disease.”

The Diabetes Self-Management Education Training Program offers both individual counseling and group settings where participants are taught how to manage diabetes and avoid its complications. The program covers the following topics:  

  • Pathology of Diabetes – What is Diabetes?
  • Medications
  • Insulin Administration
  • Insulin Pump Training   
  • Blood Glucose Monitoring
  • Nutritional Counseling
  • Exercise
  • Psychosocial Issues
  • Sick Day Rules
  • Stress Management
  • Acute and Chronic Complications
