Highland Medical, P.C., Primary Care Practices Receive Patient-Centered Medical Home Re-Certification

February 13, 2020: Highland Medical, P.C., Primary Care Practices have once again been awarded recognition by the National Committee for Quality Assurance Patient-Centered Medical Home Program (PCMH). This model emphasizes team-based care, communication, and coordination, which has been shown to lead to better care and management of chronic conditions. It also provides for better patient experience, with 83% of patients saying, “being treated in a PCMH improved their health."

“The NCQA and Highland Medical have worked over the last nine months to showcase the high quality of care and great work performed by our providers and staff every day,” said Darlene Mercieca, associate Vice President of Highland Medical, P.C. “Practices that earn recognition through NCQA have committed to providing quality improvement within their practice and a patient-centered approach to care that results in patients who are happier and healthier.”

Primary Care Medical Homes emphasizes the use of health information technology and after-hours access to resources on a patient portal, both of which improve access to care when and where patients need it. They help build better relationships between patients and their clinical care teams, which improve quality, patient experience, staff satisfaction, and reduce health care costs.
